safe-management issue

HI, having an issue with the safe-management ps script. when I run the script using the command in the readme.md
{.\Safe-Management.ps1 -PVWAURL "https://myPVWA.myDomain.com/PasswordVault" -Add -FilePath "C:\Temp\safes-details.csv"}
I get a message that the safe already exists - but it does not.... I have verified this in the vault (just to make sure I did not run it already.. or loosing my mind) -

any advise on what may be causing this?
