When I click "Manage decompilation", the window is only partly drawn. I see the title bar, the edges, a scroll bar, and some buttons. The rest (lists, text areas, background) are transparent holes in the UI. I have to bring some other app to the foreground to coax it into repainting.

I've never heard of this issue before. I would post about it at sciprogramming.com and see if anyone else has run into this problem. It might be something about your system configuration.
I'm not using Windows themes. In fact all visual effects are disabled. This is the only app I've seen this with.
As I move the mouse over text areas, they suddenly appear. The script list appears if I scroll it. It's as if they're not getting told to do an initial paint when the window is shown.
You might try building your own SCI Companion (many fixes have been made since the official release of SCI Companion - I don't know if this was fixed or not), or grabbing one that was built by someone on the sciprogramming.com forums.
